We appreciate that the fourth criteria may be confusing for some practitioners and to support you and the LADO around the new Criteria, we have introduced the consultation form. This Form Is NOT a referral form but is implemented to allow you to obtain advice and support without the need to make a referral. When completed you will need to return it to the firstname.lastname@example.org email address and we will provide the advice and complete the form, returning it back to you to ensure that you have a record and be able to record the advice given by the LADO on your systems.
